Online USAV Medical Release form

-Complete 1st Club Dues installment of $300 to confirm your position with IPV and use for Sept dues payment: You’ll receive in Invoice from IPV with instructions for payment.
–A Uniform Fitting is scheduled for Sun, Sept 11th.
-Parent Meetings will be scheduled after all tryouts are complete.
