12U Extended Season Commitment Form

We’d like to offer our 12B team the option of extending their season to Tuesday, June 10th.

-Practice: Mondays 4/28, 5/5, 5/12, 5/19; 6/2 *No practice 5/26
-Tourneys- 5/31- Cyclones, COD; 6/9-10- Asics Jr Nat’l Championship, Navy Pier
-Cost- $184
-Extended fee due by 4/28/25
-Extra cost to parents- spectator fee for tourneys; parking at Navy Pier if used.
