12U Extended Season Commitment Form

We’d like to offer our 12U’s the option of extending their season to Tuesday, June 11th.
FYI
-Practice: 4/29; 5/6, 5/13, 5/20, 5/30 (Thurs); 6/3 *No practice 5/27
-Tourneys- 6/1- Cyclones, COD; 6/1-11- Asics Jr Nat’l Championship, Navy Pier
-Cost- $200
-Extended fee due by 3/15/24
-Extra cost to parents- spectator fee for tourneys; parking at Navy Pier if used.
