Asics Championship JVA Waiver Form

Mandatory JVA Waiver: All players & coaches must complete the online JVA Waiver form by Thurs, May 29th. *This is held in your AES record so if you already completed it once this season it should be there already.

-Skip the Find Club link and click on NOT A JVA MEMBER.

-Participant Section: Enter your information as a player or coach
-Waiver Section: Read, sign, click on check boxes
-Medical Release Section: NOT Required since we already have your USAV Medical Release. You can skip this.
